The Crimson Medallion +2 is located in the southeast of the Leyndell, Capital of Ash Site of Grace. Head south after spawning at the Site of Grace and end up at sunken buildings buried in the sands. In the square between the buildings you will find an opening that looks like an entrance to the sewers. Jump down there. Do not jump directly down because if you aren’t precise then you will fall to your death.

There is a wooden ledge that you can drop to. From there, jump to the side of the wells wall. Land safely at the next ledge before dropping to the wooden beam below. You have to be as accurate as possible here as the Crimson Medallion +2 is on the wooden beam. You will see the purple item on a corpse lying across the beam.

Why can’t I find the Crimson Medallion +2?

Finding the Crimson Medallion +2 can be tricky, especially if you arrive at the location and it is not there. A common mistake with finding this piece of gear is that many people go to this location before Maliketh is defeated. The Capital needs to be destroyed first and named the Capital of Ash before you can go and retrieve this talisman.
